This program focuses on re-engaging individuals with HIV/AIDS who face accessibility, language, and financial barriers to engage in adequate, essential medical care. Services include:
- Intensive outreach efforts to high-risk individuals, clients with comorbid conditions, and clients newly diagnosed with HIV
- Client care services
- Interpretation/translation services
- Door-to-door medical transportation
- Wraparound service plan and referrals (behavioral health and drug & alcohol services)
- Medication adherence
- Assist with obtaining health insurance coverage (Special Pharmaceutical Benefits Program, Medical Assistance, ACA, Medicaid, Medicare, etc.)
